I'm quite sure that it's just like it is on osrs. And you can avoid close to 100% of the damage unless you start running towards a side where it's about to spawn (1 hit from it).
The lava pool attacks in 1 ticks on osrs, there's also some other things missing with Cerberus, but it'd make it harder. Would you like the actual Cerberus mechanics? Or the easier version on here? If you'd like to see in detail about Cerberus, or maybe some tips, you can watch this video.

It's identical to OSRS and the boss needs to be challenging.
So long as you're concentrating and moving, you won't die this way.
I've done over 500 Cerb kills on Oldschool, his lava pool doesn't seem to attack that fast imo.
